Instructions for Application Supporting Documents

Prepare an MS Word document to the attention of the
Admissions Review Committee.

The Letter of
Intent is the most important component of your
application. It helps us to determine how well your
interests match with our program offerings.
-
Write a free-form
essay of no more than 2 pages. Indicate your
aspirations, learning goals, career plans, and
specific research plans (if applicable). Highlight
any experience relevant to your program choice.

Applicants to the MPH*
- please indicate your interest in pursuing a
research or professional orientation.
*known as the MHSc degree prior to September 1,
2009.
Upload an MS Word document that includes
the following information:

education
-
employment
-
publications and presentations

teaching and research activities

other relevant experience

To confirm completion of the statistics requirement, please
complete the form as indicated with the following information:
-
the university at which you completed the course (or
courses) that satisfies this requirement
-
the course code and title
-
a URL (if available) containing the course outline
-
the session in which you completed the course
-
your grade (% or letter grade)
Please scan your transcript in grayscale at the lowest
dots per inch (dpi) that results in a legible document.
We recommend 72 dpi whenever possible.
If your transcript is double-sided, please be sure to scan the front and back of each page.
The transcript legend must appear at least once.
You must upload your transcript .pdf format.
A .jpg file must be converted to .pdf format to be uploaded.
For Macintosh users, please note that the filename must include the appropriate 3 letter extension.
Do not upload a document that is password-protected or that contains macros.
Scanned documents must be clearly legible and print on standard U.S. 8 1/2" x 11"
paper. The admissions committee will make its decision about your application based on this scanned transcript.
Please ensure that the file is complete and readable before submitting.
If you have more than one transcript, scan each individually to
be uploaded to the application website.
The transcripts should be uploaded in chronological order.
If you have attended a Canadian university, you may submit a
.pdf file of your academic history from the university's Student Web Service
(ie; ROSI at U of T) in place of a transcript. The file
should contain the university's grading legend and your
name. If the admissions committee determines that the file is incomplete you may be asked to
upload a copy of the transcript. If you have attended multiple institutions, the other universities’ transcripts (with legend) must still be
included in your application.
The file size of each scanned transcript to be uploaded must not exceed
3 megabytes (MB) in size.
Non-Canadian or
Non-US institutions
-
If you attended a
University outside of North America, scan and send your transcripts as
above. We have
reference sources that provide degree equivalencies
from all over the world, so no explanatory notes
should be necessary.

For Non-English
transcripts, a notarized English translation must
also be scanned and included with your uploaded
document. Be sure to keep a copy of the
translation for your records.

If requested, you may still need to
provide official transcripts sent directly from the
issuing institution to U of T in a sealed envelop,
as instructed in this application.
Applicants will be notified by the Dalla Lana
School of Public health, through the on-line application system,
if their OFFICIAL TRANSCRIPTS are required. If requested,
official transcripts are required from
EACH university you have attended. They
must be sent directly to:

from the issuing
university in a sealed envelope.

Two Letters of Reference
Select referees who are familiar with your academic and
career aspirations. Your referee should be able to
comment on your academic ability, and potential to
succeed in a graduate program, as well as a Public
Health professional or researcher. Generally, a former
professor/supervisor or employer is the most appropriate
referee. Your referees will be contacted via email by the School of Graduate Studies at the University of
Toronto, and asked to submit a reference form in
addition to a more detailed letter.
It is critical that you contact your referees in advance
to explain this process, so that they can comment
appropriately. Also, it may be necessary to follow up
with your referees, to ensure that the reference is
submitted, electronically, by the deadline.

English Proficiency
Proficiency in the English language is required and must be
established by all applicants from universities outside
of Canada where the native language is not English.
Note that a university education in English may not be
considered sufficient (e.g. TOEFL/TWE is still required for
graduates from such countries as India, Pakistan and Bangladesh).
Your application will not be considered without
proof of
English language proficiency. A satisfactory English
proficiency test result must be available prior to the
application deadline. The test score can be no more than two years old.
Please indicate your score on your application AND request that an original test score be sent
directly from the testing service to the Graduate
Department of Public Health Sciences in the Dalla Lana
School of Public Health.

If you have received
government or agency support for your studies, please
ask the funding agency to send a letter directly to the
Graduate Office,
indicating the details of the funding which it will
provide for you academic program.
International applicants should contact their own
government or Canadian Embassy Consulate for information
on sponsorship through international agencies and
Canadian government programs.
